Safety Weekend and Night Tool (SWAN)
The
SWAN is best scored with a group of clinical and administrative
leaders who complete the assessment together.
Score only those questions that
are applicable to your Hospital. (For example, if you do not
have a Cath Lab score these as N/A). Then take the number of
“Yes” Responses/Total Number of applicable questions and
multiply by 100 to obtain your Final Score.
